Clinical Pharmacist Intervention Note – Integration Form

As a Clinical Pharmacy Inspector, I identified the lack of a designated space within the patient’s medical file for clinical pharmacists to document therapeutic recommendations and report medication-related issues. To address this gap and ensure proper communication with physicians, I developed and implemented a structured Pharmacist Intervention Form to be attached directly to the patient’s chart. This form allows clinical pharmacists to formally record their observations, including medication errors and evidence-based therapeutic suggestions, facilitating improved multidisciplinary collaboration and enhancing patient safety. The form was standardized and distributed across all hospitals under my supervision, ensuring consistent documentation and acknowledgment of the pharmacist’s role in the care process.
